For your convenience and to expedite your visit with us, we have provided our intake form so you can print and fill them out prior to your appointment.
On the day of your appointment, please bring any eyewear or sun-wear that you use, including over the counter readers. If you currently wear contact lenses, please bring any boxes or vials with you that may help us to determine what you have previously worn. It is very important that you include all of the prescription and over the counter medications that you are currently taking (including eyedrops) on your intake forms for the doctor to review, as many of these may affect your eyes.
