For your convenience and to expedite your visit with us, we have posted our intake form below so you can print and fill it out prior to your appointment.  Our privacy policy as required by HIPPA is also posted for your review.  On the day of your appointment, please bring any eyewear or sun-wear that you use, including over the counter readers. If you currently wear contact lenses, please bring any boxes or vials with you that may help us to determine what you have previously worn. It is very important that you include all prescription and over the counter medications that you are currently taking (including eyedrops) on your intake forms for the doctor to review, as many of these may affect your eyes.

Please try to give us 48 hours notice if you need to reschedule or cancel your appointment as our schedule usually fills up one to two weeks in advance.
